MINOR PERFORMER RELEASE AGREEMENT
To Whom It May Concern:
As parent/guardian of Hunter Minor, I understand that this Performer Release is provided to Dreadhead Films/ImageFlick (“the Filmmaker”) with respect to his participation in the performance of the composition A Mile Away (the “Performance”) to be featured in the motion picture tentatively entitled Foreshadow (the “Picture”).
By signing below, I authorize and grant, without limitation, a non-exclusive worldwide perpetual license to reproduce, record, publish, or otherwise copy such Performance and to broadcast, display, reproduce, edit, exhibit, and distribute the Performance over the Internet, television, cable, or any other communication medium now existing or hereafter for the purpose of the promotion, production, and distribution of the Picture.
I hereby certify and represent that I am over 18 years of age and have read this performer release agreement and fully understand the meaning and effect thereof. Further, I understand that my typed name below and email submission of this agreement will formally serve as my signature.
